This post will be a little rough I basically wanted to get you the links i found which were a great read and i think i good resource for information and even same invoices and such. If you've done any marketing it may be info you are already familiar with and perhaps you have already done this research, i dunno. If you have, then just skim and mostly ignore. I simply wanted to share what I found and I felt this method would be less whelming than a long winded and annotated text.
The resource I found is below my notes and quotes, which is up right after the third page break. And really I'll break it up because 2/3 is directly from the site i referenced below. We can fully incorporate anything you want into the site, I intended upon finding more expensive solutions and comparing them to your rates on an addendum page but everybody wants you to have them come over to make an individual quote. One thing that can be done is to describe the quote process, that it'll take 1/2 hour & what to expect etc. Business cards can be done for $40 for 500 at vistaprint.com i've used those guys a bunch and love their rates. As mentioned flyers and other simple things can be printed off at the library for free and I've got many examples I can show of my previous work making trifold and pamphlet mailers and things along that nature. If there's a link here it's because I threw a bunch on my google drive and made a quick page as a reference.

Forms: Yea, I'm sorry but other than straight writing a page of HTML (which I may try but I am reticent to do) I don't know how to do that. I may buy a solution because unless I figure it out, I'll need some way to get my mailing list form accomplished. I worked on it a bit in March but got sidetracked.
It seems as though $40-50 per hour is the right range but you may get more profit from a by the sq ft quote. I've included my pull quotes and snippets I copied right here. Though I should note that people are out there and can be found at care.com that work from 13-17 per hour. Not that you should, but that the exist..

Flat fee house cleaning pricing guide
A weekly or bi-weekly clean can be $100–170 for a typical single-family home. You can raise your flat fee if you’ll be cleaning more rooms or visiting less often, or lower your rate if you’re cleaning an apartment or smaller home.
The average room rate for a one-bedroom, one-bathroom home is $75–110, plus $25 for each additional bedroom and $10 for each extra bathroom.
So if you assume a starting rate of $95 and you’re cleaning a three-bedroom, two-bathroom home, a reasonable rate could be $160 for a weekly cleaning job.

How much do I charge to clean a house per square foot?
For weekly or bi-weekly cleaning, you can charge 5–15¢ per square foot. As a general rule, the higher the square footage, the lower your rate should be.
If you’re doing a move-out clean, you can charge up to 22¢ per square foot since you’re actually cleaning every single one of those square feet.
One-time deep cleaning can cost up to 33¢ per square foot since it might not have been done for a while, and you’re putting more effort into each square foot.
So if you’re deep cleaning a furnished 700 sq ft apartment at a square footage rate of 30¢, you would quote $210 for that one-time job.
Regularly cleaning a 2,000 sq ft home at a rate of 7¢, on the other hand, would earn you $140 each week. It’s less income per job, but it’s recurring work that you can always count on.

Specialty services house cleaning pricing guide
Floor cleaning services are most often based on square footage, but you could also charge hourly, flat fee, or room rate. Buffing is 4–12¢, tile cleaning is 12–21¢, carpet cleaning is 16–28¢, and stripping and waxing is 30–50¢.
The average cost of appliance cleaning depends on how large the appliance is and how much work is needed. You can usually charge $10–35 per appliance, but you could quote up to $100 for a larger job like a fridge cleaning if it’s in really bad shape.
